PLEASE READ CAREFULLY.
In consideration of granting permission for my minor child to participate in events and/or activities involving paintball, low-impact paintball, or airsoft, organized by 9059-2643 Québec Inc. (hereinafter referred to as "Jeu Warzone Paintball"), I acknowledge and agree to the following terms:

1. Acknowledgment of Risks
I acknowledge that I have been informed of the risks associated with these activities, and I understand they involve inherent dangers. I accept that my minor child, named above, may be exposed to risks of bodily injury, permanent disability, or even death by participating in these activities or being present at the location—whether as a participant, spectator, companion, volunteer, or in any other capacity.
2. Voluntary Participation
Despite these risks, I authorize my minor child to participate in the activities organized by Jeu Warzone Paintball. I certify that they are in adequate physical, mental, and emotional condition to participate, that their protective equipment is appropriate and in good condition, and that they will not be under the influence of alcohol or drugs during participation.
3. Associated Risks:
I understand that participation involves certain risks, including but not limited to: Falls, natural obstacles (such as ditches, streams, tree branches); equipment malfunction or failure.
I acknowledge that absolute safety cannot be guaranteed, and accidents may occur despite all precautions taken.
4. Agreement to Follow Rules
I agree that my child will comply with all safety and conduct rules, both during and outside of game play.
5. Awareness of Specific Rules:
I have read and understood the following safety rules: no running in stairways or on slippery surfaces; no climbing on unauthorized structures; children under the age of 7 are not permitted to participate; minors must be supervised by a responsible adult other than the staff of Jeu Warzone Paintball.
6. Use of Photos and Videos:
I authorize Jeu Warzone Paintball to use any photos or videos taken during events for promotional purposes.
7. Clothing and Equipment :
I agree that my child will wear: appropriate clothing for the activity that may get dirty or damaged; suitable footwear such as hiking boots or running shoes. I understand that high heels, sandals, or cleats are not suitable or allowed.
8. Responsibility for Loss or Damage :
I understand that my child must keep track of any equipment provided. In the event of loss, damage, or theft of said equipment, I agree to cover the cost of replacement.
9. Safety During Game Play Wearing
a mask is mandatory at all times, including when outside of the game area or after the game has ended. If there is any issue with the mask, my child will request assistance in a designated safe zone.

THEREFORE, I AGREE TO THE FOLLOWING:
A. Acceptance of Risk :
This waiver constitutes an ACCEPTANCE AND ASSUMPTION OF RISKS associated with my child’s participation in events and activities at Jeu Warzone Paintball, even if those risks result from negligence, gross negligence, or improper emergency procedures by Jeu Warzone Paintball or any party affiliated with the event.
B. Release of Liability:
I waive, on behalf of my minor child, any and all claims against Jeu Warzone Paintball and its officers, employees, and affiliates for any loss, injury, or damage arising from participation in the events or activities, including those caused by negligence, breach of contract, or failure to uphold a duty of care.
C. Indemnification:
I agree to indemnify and hold harmless Jeu Warzone Paintball and its affiliates from any claims, legal fees, or damages resulting from legal action related to my child’s participation in the events or activities.
D. Governing Law:
This agreement is governed by the laws of the Province of Québec. I confirm that I have read, understood, and agree to the full content of this document.
