PLEASE READ CAREFULLY.

In consideration of granting permission for my minor child to participate in paintball, low-impact paintball, or airsoft events and/or activities organized by 9059-2643 Québec Inc. ("Jeu Warzone Paintball"), I acknowledge and agree to the following terms:

1. Acknowledgment of Risks:

Having been informed of the risks associated with the mentioned activities, I acknowledge that they involve inherent dangers. I therefore accept that my minor child may be exposed to risks of bodily injury, permanent injury, or even death.

2. Voluntary Participation:

Despite the identified risks, I authorize my minor child to participate in the activities. I certify that they are in adequate physical and mental condition to participate.

3. Associated Risks:

I understand that these risks include, but are not limited to:

• Falls

• Natural hazards (ditches, streams, tree branches)

• Equipment malfunctions

4. Commitment to Follow Rules:

I commit to ensuring that my child follows all safety rules and instructions at all times.

5. Specific Rules:

• Running in hazardous or unauthorized areas is strictly prohibited

• Climbing on unauthorized structures is strictly prohibited

• Children under 7 years of age are not permitted

• Mandatory supervision by a responsible adult

6. Photos and Videos:

I authorize the use of photographs and video recordings for promotional purposes.

7. Clothing and Equipment:

I agree to provide appropriate clothing and suitable footwear for the activities.

8. Responsibility:

I understand that I am held financially responsible for any loss or damage caused to the equipment.

9. Safety:

Wearing a protective mask is mandatory at all times on the field.
